چکیده

The timing of surgery has been a major problem in the management of chronic severe mitral regurgitation (MR). In adults, because of poor prognosis of postoperative left ventricular (LV) dysfunction, sequential echocardiographic assessment of LV function for optimal operation time is recommended. 2 However, in children, therapeutic guidelines of chronic MR are less well defined and severe symptoms are often used as the indication for surgery. Furthermore, report on the change of LV function after mitral valve (MV) surgery is very limited. Therefore we analysed the records of children who had MV surgery for severe MR to evaluate the change of LV function and to identify any potential risk variable for late postoperative LV dysfunction.

Prediction of outcome after valve replacement for rheumatic mitral regurgitation in the era of chordal preservation.

BACKGROUND Noninvasive predictors of important outcomes after valve replacement for mitral regurgitation have not been examined in a rheumatic population (in whom the results of valve repair are suboptimal) in the era of chordal preservation. Timing of valve replacement thus remains a difficult question in rheumatic mitral regurgitation. METHODS AND RESULTS Of 278 patients followed after valv...

Doppler-Derived Rate of Left Ventricular Pressure Rise

A new Doppler-derived index of the rate of left ventricular (LV) pressure rise (AP/At) was evaluated for the prognostic stratification of patients with chronic mitral regurgitation. The index is derived from the continuous wave Doppler mitral regurgitation signal by dividing magnitude of LV-left atrial pressure gradient rise (Ap) between 1 and 3 m/sec of the mitral regurgitation velocity signal...

Clinical and Prognostic Impact of a New Left Ventricular Ejection Index in Primary Mitral Regurgitation Because of Mitral Valve Prolapse.

BACKGROUND To prevent left ventricular dysfunction (LVD), surgery is recommended in patients with severe primary mitral regurgitation as soon as ejection fraction (EF) ≤60% or LV end-systolic diameter ≥40 mm. However, LVD may be concealed behind preoperative normal LVEF and LV end-systolic diameter. We sought to identify whether a new composite echocardiographic Doppler marker of the LV ejectio...

Doppler-derived rate of left ventricular pressure rise. Its correlation with the postoperative left ventricular function in mitral regurgitation.

A new Doppler-derived index of the rate of left ventricular (LV) pressure rise (delta P/delta t) was evaluated for the prognostic stratification of patients with chronic mitral regurgitation. The index is derived from the continuous wave Doppler mitral regurgitation signal by dividing magnitude of LV-left atrial pressure gradient rise (delta p) between 1 and 3 m/sec of the mitral regurgitation ...

اثر PercutaneousCoronary Intervention Elective بر بهبودی نارسایی ایسکمیک میترال

Introdution: Ischemic mitral regurgitation (IMR) remains one of the most complex and unresolved aspects of ischemic heart disease that the impact of percutaneous coronary intervention (PCI) on improvement of intensity of ischemic mitral regurgitation is not well clarified. Patients with coronary artery diseases and ischemic mitral regurgitation have a worse prognosis than the patients with coro...
